Your First Visit
Please arrive about 10 minutes early so that you have enough time to fill out a New Patient form. If you prefer, you can print and complete the form at home using the link below and bring it to your appointment.
Initial Examination

Treatment Plan

Payment Methods
The full treatment cost (or the patient portion if direct-billing to your insurance) must be paid at the time of your appointment. We accept payment by cash, debit or most major credit cards.

Where is your clinic located?
Our clinic is located at 120-40 Lakewood Blvd., beside Lakewood Medical Centre. The best way to get to us is to take Fermor Avenue.
We offer extended hours Mondays through Thursdays to make it easier for you to prioritize your oral health.
